The feedback activity module () enables a teacher to create a custom survey for collecting feedback from participants using a variety of question types including multiple choice, yes/no or text input. It is similar to the choice activity module with the primary difference being that you can have multiple questions.

Feedback responses may be anonymous if desired, and results may be shown to all participants or restricted to teachers only. Any feedback activities on the site front page may also be completed by non-logged-in users.

Note:  Feedback activities should not be used for course evaluation as a dedicated system is already in place for course evaluations.

Setting up a feedback module is a two step process. The first step is naming the feedback and choosing the general options (see Feedback: Settings). The second step is entering the list of questions to include in the feedback (see Feedback: Questions).
